Management of astigmatism post penetrating keratoplasty

Despite improvements in outcomes of penetrating keratoplasty, postoperative astigmatism remains a challenge. This review describes the various techniques used to minimize postoperative penetrating keratoplasty astigmatism. The advent of descemet stripping endothelial keratoplasty for corneal disorders involving the endothelium has decreased the incidence of postoperative astigmatism. Spectacles, contact lenses, and refractive surgical techniques are used in various situations to manage high postoperative astigmatism and improve functional vision. Refractive surgical procedures discussed in this review include excimer laser photorefractive keratectomy and laser in situ keratomileusis, mechanical and femtosecond laser incisional keratotomy, wedge resection, and intraocular lens implantation

A importância da sutura com 24 pontos no transplante de córnea / Evaluation and importance of 24 knots interrupted suture on corneal transplantation

Realizamos um estudo retrospectivo em 231 olhos com ceratocone que foram submetidos a transplante penetrante de córnea para avaliarmos os resultados obtidos em relaçäo ao astigmatismo e à acuidade visual. Os pacientes foram divididos de acordo com a técnica empregada em 3 grupos: grupo I: 27 pacientes com 8 pontos separados e 1 contínuo; grupo II: 158 olhos com 16 pontos separados e grupo III: 46 olhos com 24 pontos separados. Quando comparamos o astigmatismo entre os grupos houve uma diferença estatisticamente significativa entre o grupo I (sutura contínua) e o grupo II (16 pontos) no segundo mês pós-operatório cujos valores foram: 4,09 +ou- 3,37 e 6,21 +ou- 2,99 (p=0,05) e também entre os grupos II e III no segundo ano pós-operatório com valores iguais a: 6,14 +ou- 2,65 e 3,64 +ou- 1,94 (p=0.03). Em relaçäo à acuidade visual, houve diferença significativa entre os grupos I e III no primeiro ano pós-operatório com os seguintes valores: 0,58 +ou- 0,21 e 0,74 +ou- 0,22 (p+0,03). A sutura que menos induziu astigmatismo foi a de 8 pontos separados e um contínuo proporcionando a recuperaçäo mais lenta da AV porém, proporcionou maior estabilidade no astigmatismo, garantindo o melhor resultado final, inclusive em relaçäo à acuidade visual

Análise do astigmatismo com incisäo pós-limbar e sutura mista / Study of the induced astigmatism with posterior limbar incision and mixed suture

Os autores apresentam uma análise do astigmatismo induzido, após a cirurgia de catarata com incisäo pós-limbar e sutura mista. Foram estudados 35 olhos aleatoriamente, de um total de 160 cirurgias realizadas pelo cirurgiäo, com idades que variavam de 49 a 92 e média de 72 anos. Foi realizada ceratometria no pré-operatório, com 60 dias e com 90 dias pós-operatório e ceratoscopia no trans-operatório, utilizando o disco de Shepard

Técnica quirúrgica de la querotoplastía penetrante / Surgical technique of the penetrating keratoplasty

Este trabajo describe las técnicas en boga para realizar una querotoplatía penetrante, incluyendo cuidados pre y postoperatorios, técnicas de trepanación del donante y receptor, como también tipos de suturas y algunas maniobras para disminuir el astigmatismo postoperatorio

Astigmatismo induzido pela cirurgia da catarata: sutura de nylon x poliéter / Surgically induced astigmatism after cataract extraction: nylon x poliester sutures

Foram analisados dois grupos de 20 pacientes com a finalidade de ser observada a evoluçäo e a magnitude do astigmatismo induzido no pós-operatório da cirurgia da catarata suturada com fios de nylon ou poliéster. Tal comparaçäo demonstrou que o astigmatismo induzido nos grupos nylon ou poliéster näo diferiu estatisticamente durante o período estudado. O grupo nylon mostrou um decréscimo do astigmatismo induzido significantemente maior que o grupo poliéster no período entre 90 e 180 dias. Os fios estudados parecem induzir astigmatismos com evoluçöes distintas no pós-operatório

The posterior chamber intraocular lens, the crowning of modern extracapsular cataract extraction

The modern extracapsular cataract extraction [E.C.C.E.] is not at all similar to the old, well-known extracapsular technique, which had been easily conquered by the intracapsular extraction [I.C.C.E.], we have all been brought up to do intracapsular surgery. Even the youngest generations are still trained to do I.C.C.E. Intraocular lens Implantation is far safer and much less complicated in conjunction with E.C.C.E. The posterior chamber [whole retropupillary] lens is far superior to the anterior chamber one and is only possible with E.C.C.E. We have Implanted 84 posterior chamber lenses, of the modified J-loop, after E.C.C.E, and the results have been excellent. The advantages were: 1- Much less incidence of vitreous loss 2- Less astigmatism with the small posterior limbal incision 3- The absence of intraocular pressure elevation 4- The excellent visual acuity and the absence of bullous keratopathy, due to the very soft ocular tension and the use of Na hyaluronate to maintain the A.C. Very few cases of postoperative iridocyclitis and a few cases of capsular thickening were controlled in the usual way
